SPORT PHYSICALS FOR FALL SPORTS 2020-2021

Parents,- Please email the LWE Athletic Secretary- Debbi Rossi- drossi@lw210.org your student's Sport Physical by August 1st to insure they are all set for Fall try outs. Fall try-outs begin August 10th.

Freshman do not require a separate Sports Physical- the High School Physical required to enter school is the only physical they need. A copy of that Freshman Physical needs to be emailed or mailed directly to the Athletic Office. (In addition to the Nurse)

Sophomore, Juniors and Seniors, require a Sports Physical.https://s3.amazonaws.com/pubtemp/live/content/LWE/documents/20170412014051.pdf

The Sports Physical must be scanned (do not send a picture). Parent and Student signature is required on the first page.
